The Community Innovations Initiative aims to promote older peoples well being and independence, and prevent their need for statutory services by linking them with opportunities in the community.
We are looking for creative and enthusiastic individuals who can make a difference in helping older people get the most out of life. You will have experience of working with older people and/or disability in a health or social care setting. An NVQ level 2 in Care is desirable.
You will work in a multi agency/multi disciplinary team working to support the Nurse, Social Worker and Occupational Therapist to put plans in place with older individuals. This requires some degree of autonomous working, good time management and basic admin skills; i.e. using diaries, e-mail, internet; computerised notes and telephone.
Job purpose
To work within a multi-disciplinary and multi- agency Innovations Locality Team in delivering support in an enabling manner to compliment well being and prevention programmes for older people in their own homes and the wider community.

Essential knowledge, skills & experience:
- It is essential that you have experience of working with older people in a social or health care setting.
- Effective communication skills both verbal and written.
- Evidence of sound literacy & numerical skills
- The ability to work on own initiative and unsupervised
- Possess proven coaching and mentoring skills
- Demonstrate good observation, listening and feedback skills with the ability to seek advise from other appropriate professionals.
- Demonstrate the ability to work in partnership with other organisations
- Able to demonstrate an understanding of social care values; choice, privacy, dignity
- Team player
- Personal integrity, common sense approach and non-judgemental attitude
- Demonstrates ability to prioritise delegated workload and effective time management skills.
- Understanding of the needs of older people, particularly in relation to prevention and wellbeing programmes
- Ability to fulfil the travel requirements of the post.
